Guide to Filling Out the Personal Data Sheet (PDS) | Downloadable CSC Form No. 212

Personal Data Sheet (PDS) can be filled out using either MS Word or MS Excel formats. For Excel, tick boxes will automatically mark once clicked. Ensure that the PDS is signed and dated at the bottom of each page. Please follow the instructions below to accurately complete each field in the Personal Data Sheet (PDS).

General Instructions:

  • Format: Entries can be typed or handwritten. If handwritten, use block capital letters (e.g., PRINT) with a pen.
  • Accuracy: Provide accurate information and avoid leaving any fields blank. Use “N/A” if a field is not applicable.
  • Signature: The PDS must bear your signature and the date of accomplishment at the bottom of every page.
  • Additional Sheets: If applying for a position, include an additional sheet for work experience relevant to the position.

I. Personal Information

  • Name: Enter your name in the format: surname, first name, name extension (if any), middle name. Use one space for each character or letter.
  • Dates: Use numeric format: mm/dd/yyyy.
  • Civil Status: Provide specifics if choosing “Others.”
  • Agency Employee Number: This is your ID number in the current agency.
  • Citizenship: For foreign/dual citizenship, select or type the foreign country of birth/naturalization from the dropdown list.

II. Family Background

  • Names: List names of spouse and parents in the format: surname, first name, name extension (if any), middle name.
  • Mother’s Name: Use her maiden name (name before marriage).
  • Children: List full names (first name and surname) of all your children.
  • Date of Birth: Use numeric format: mm/dd/yyyy.

III. Educational Background

  • School Names: Enter full names of schools; do not abbreviate.
  • Educational Levels:
    • Elementary Level: Indicate PRIMARY EDUCATION if graduated.
    • Secondary Level: Specify HIGH SCHOOL (old curriculum) or JUNIOR HIGH SCHOOL/SENIOR HIGH SCHOOL (K-12 curriculum).
    • College: List full names of degrees (e.g., ASSOCIATE IN ARTS, AB ECONOMICS, BS PSYCHOLOGY, MA IN HISTORY).
    • Postgraduate: List any master’s or doctorate degrees.
  • Graduation Information: Indicate the year of graduation if applicable. If not graduated, state the highest grade/level/units earned.
  • Attendance Period: Use school years format (e.g., 1992-1996).
  • Honors: Include any scholarships and academic honors received at each level.

IV. Civil Service Eligibility

  • Eligibility: List all civil service eligibilities earned with ratings, dates, and places of examination/conferment.
    • Example:
      • Career Service Sub-Professional EO132/790 – Veteran Preference Rating
      • Career Service Professional PD 907 – Honor Graduate
      • Career Service Executive RA 7883 – Barangay Health Worker
      • Stenographer Barangay Official PD 997 – Scientific and Technological Specialist
  • License: If eligibility includes a license (RA 1080), provide the license number and validity date.

V. Work Experience

  • Positions: List all positions held, both public and private, starting with your current job.
  • Dates: Use numeric format: mm/dd/yyyy.
  • Details: Provide full position titles and complete names of departments/agencies/offices/companies. Avoid abbreviations.
  • Salary: State the monthly salary in figures (e.g., P21,877).
  • Salary Grade and Step: Use the format “00-0” (e.g., 24-2).
  • Employment Status: Indicate status (e.g., permanent, temporary, casual, contractual).
  • Government Service: Mark “yes” if the position is in public/government employment or “no” if private.

VI. Voluntary Work or Involvement in Civic/Non-Government/People/Voluntary Organizations

  • Organization: List the full name and address of the organization where you volunteered.
  • Dates: Use numeric format: mm/dd/yyyy.
  • Hours: Indicate the number of hours volunteered.
  • Position/Nature: Specify the position or nature of voluntary work.

VII. Learning and Development Interventions

  • Titles: List full titles of learning and development (L&D) interventions attended, starting with the most recent.
  • Dates: Use numeric format: mm/dd/yyyy.
  • Hours: State the number of hours attended.
  • Type: Indicate the type of L&D intervention (e.g., managerial, supervisory, technical).
  • Institution: Provide the full name of the institution/agency that conducted or sponsored the program. Do not abbreviate (e.g., CSC should be Civil Service Commission).

VIII. Other Information

  • Skills/Hobbies: List any special skills or hobbies.
  • Distinctions/Recognition: Provide details of non-academic distinctions or awards received.
  • Memberships: List memberships in professional associations/organizations with their full names.

Additional Information

    • Questions #34-40: Provide responses to questions 34 through 40 on the right side of the sheet. Include details or specifications for any “yes” responses.
    • Question #41: Provide the full names of references in the format FIRST NAME, MI, SURNAME, along with their addresses and telephone numbers.
    • Question #42:
      • Affix your signature and right thumb mark in the boxes provided as confirmation of the PDS completion.
      • Indicate your government ID number and date of issuance in the designated boxes.
      • Attach a recent passport-sized picture (4.5 cm x 3.5 cm) with a complete handwritten name tag and signature over printed name. The picture must be taken within the last six months; computer-generated or photocopied pictures are not acceptable.
